ALL students (regardless of age) participating in activities at Amelia Art Studio will be required to sign the following documents. These documents are administered once per year in beginning in August and kept on file as valid through the following August.
The only portion of the document you may decline and still be permitted to participate in Studio activities is the Photo Waiver.
I, the undersigned, do hereby grant to Amelia Art Studio, LLC, (hereinafter “Amelia Art Studio”) and their photographer or videographer permission to take photographs and/or videos of myself and any of my minor children (as named below, if applicable). I also give Amelia Art Studio permission to use, edit, copy, distribute, transmit, reproduce, sell, reformat or publish any photos, still or moving, my name and/or any likeness of me or my minor children (as named below, if applicable) for any legitimate uses at any time now or in the future that it may deem proper, and that Amelia Art Studio shall be the exclusive owner and hold the exclusive copyright therein. I agree that I will not have a right to examine or approve any use of any photos, still or moving, my name and/or any likeness of me or my minor children and that I will not be compensated for such use. I do further waive any right in or to any such photos and release Amelia Art Studio from any and all liability, claim or demand whatsoever, for the use of any photos, still or moving, my name and/or any likeness of me or my minor children. I am 18 years of age or older and have the right to contract on my own behalf and on behalf any minors named below.
Although the safety and well being of your child(ren) is our top priority at Amelia Art Studio, we, (Lauren Templeton-business owner, building owner, volunteers, consultants, and staff) will not be responsible for and will be forever RELEASED from any injury(ies), claims, and/or any other liability arising from your child’s attendance and participation in our services and programs at Amelia Art Studio, and/or for any lost property left behind by your child. By signing this waiver form, you agree to indemnify and hold harmless from any and all injury(ies), claims, causes of action, and/or any other liability arising from your child’s attendance and participation in our services and programs the above-mentioned RELEASEES at Amelia Art Studio. You further affirm that you understand the contents of this agreement and are free to choose not to participate in said program or make use of said services. By signing this waiver you affirm that you have decided to allow your child to participate in programs and services provided at Amelia Art Studio with the full knowledge that the RELEASEES will not be liable to anyone for personal injuries and property damage your child or you may suffer while at Amelia Art Studio. Thank you for your support. It is a pleasure to welcome your child to Amelia Art Studio.
I understand that COVID-19 (Coronavirus) is considered to be extremely contagious and can result in a range of symptoms, which include, but are not limited to, fever, shortness of breath, fatigue, loss of taste or smell, and nausea or vomiting. I acknowledge that COVID-19 is primarily spread by person-to-person. A person does not have to be showing signs of illness in order to spread this virus. I understand that the risk of person-to-person spread of the virus is increased by close physical contact. I acknowledge that participation in Amelia Art Studio classes could increase the risk of transmitting COVID-19.
I affirm that I have not been diagnosed with, demonstrated any symptoms of or have in any way been exposed to any communicable diseases (including but not limited to the virus commonly referred to as COVID-19) within the past 14 days. I also affirm that I will adhere to all safety precautions communicated by the teaching staff when engaging in Amelia Art Studio classes.
By signing this Agreement, I acknowledge that I understand the risks related to COVID-19 and other communicable diseases and understand that the risk of contracting COVID-19 may be increased by participation in Amelia Art Studio classes. I voluntarily assume the risk of allowing my child to participate in Amelia Art Studio classes.
By signing this Agreement, I acknowledge that I have read the above and understand the contents of the Waiver. I acknowledge the risks associated with participation in Amelia Art Studio classes and the possible contraction of COVID-19 or other communicable diseases and want my child to participate in Amelia Art Studio classes.
